Our Technology Platform combined with skillful Care Coordination allows us to provide Chronic Care Management, Remote Patient Monitoring, and Transitional Care Management at scale, including both the patient and their care network (family/caregivers/physicians) via multiple communication methods. We offer literally a private labeled, turnkey service delivered to patients on behalf of our clinical partners (practices, hospitals, senior communities). The result is clear and impactful: Health and wellness improvements, more involvement from the care network (through a Personal Health Record and Comprehensive Care Plan), and better access to life-saving clinical care for those who have been disproportionately affected by COVID-19 (lower-income individuals and seniors of color).

Each year almost $4 Trillion (literally 90% of the nation’s annual healthcare expenditures) is spent on chronic and acute disease management. Yet with over 44 million Americans eligible for a Medicare-reimbursed Chronic Care Management program, fewer than 1.5% take advantage of this federal initiative. Often the patient’s healthcare practitioner isn’t staffed to be able to deliver this needed service. And COVID-19 has exacerbated the problem, as fewer than 40% of Seniors in the U.S. had a clinical encounter (in person or by telehealth) during the pandemic, and mental health issues (anxiety, social isolation, loneliness) are on the increase in the senior population. That’s where 1TH-CM finds our opportunity.
1TH-CM is currently serving patients in transition with Congestive Heart Failure from 42 hospitals across 3 states (Texas, Mississippi, and Tennessee). We are scaling toward serving 7,500 patients per month with this contract. Our latest contract is with a Respiratory Therapy company to serve 17,000 patients in Southern California. We also have signed multiple Referral Partner agreements including with C3HIE (the largest Health Information Exchange in Texas serving 57 hospitals and hundreds of clinical practices across the state.

1 True Health - Care Management has secured a Lead Investor for a priced equity round. We are raising $2.25M on a pre-money valuation of $7 Million. The lead investor and two limited partners have invested $800K toward this $2.25M goal and we are actively seeking additional limited partners to close the round.
1TH-CM Funding:
1 True Health has obtained a contract with Advanced Homecare, a Respiratory Therapy company focused on sleep apnea serving 17,000 patients in Southern California. There are over 4,000 Respiratory Therapy DME companies in the U.S.
1 True Health inked a Referral Partner Agreement with a company focused on Durable Medical Equipment (DME) providers. This is an exciting new market for 1 True Health as it is an ”untapped” source of patients suffering from chronic/.acute conditions.
By December of 2023 we had signed on five (5) organizations as Referral Partners, serving clinical practices, hospitals, and senior living communities. This includes C3HIE, the largest health information exchange in the state of Texas.
