Each year over $3 Trillion (literally 90% of the nation’s annual healthcare expenditures) is spent on chronic and acute disease management. Yet with over 44 million Americans eligible for a Medicare-reimbursed Chronic Care Management program, fewer than 1.5% take advantage of this federal initiative. Often the patient’s healthcare practitioner isn’t staffed to be able to deliver this needed service. And COVID-19 has exacerbated the problem, as fewer than 40% of Seniors in the U.S. had a clinical encounter (in person or by telehealth) during the pandemic, and mental health issues (anxiety, social isolation, loneliness) are on the increase in the senior population. That’s where 1TH-CM finds our opportunity.

1TH-CM initially focused on the Austin, TX area. We quickly enrolled patients through a B2C model. In late 2022 we launched a B2B sales effort and we are now forming partnerships with Clinical Practices and Hospitals in Austin, Dallas, and in rural Texas. We have also inked a Reseller Agreement with the largest Health Information Exchange in Texas covering 82 counties, 57 hospitals and hundreds of clinics and physician groups. We have also been endorsed by the Texas Organization of Rural & Community Hospitals (whose members include 163 rural hospitals in the state of which 86 are considered to be Critical Access Hospitals, where care is very limited). We have also signed a Reseller Agreement with a Transitional Care Management company serving 16 hospitals over 4 states. And 1TH-CM has begun selling to Senior Living Communities offering clinical services to their residents.
