Product Release: trueCare Patient Engagement platform

1 True Health released a mobile and cloud-based Patient Engagement application (“trueCare”) that allows us to provide Chronic Care Management, Remote Patient Monitoring and Transitional Care Management on behalf of clinical practices, hospitals, and senior living communities (addressing the issues of staffing and patient engagement). Our technology connects the senior with their care network (family/caregivers/physicians) via multiple communication methods and provides meaningful information on the patient’s past encounters and real-time health information. trueCare also visualizes the senior’s care plan instructions and engages them via notifications and visual/verbal communication to improve compliance.
